Acceptance of Terms
By accessing our website, scheduling an appointment, or using the services provided by Primary Medical Services, you agree to be bound by these Terms of Service. If you do not agree with any part of these terms, please do not use our services. We reserve the right to update or modify these terms at any time.
Your continued use of our services following the posting of changes constitutes your acceptance of such changes.
Our Services
Primary Medical Services provides primary care services including:
- Annual physicals and preventive health examinations
- Chronic disease management and treatment
- Urgent care for acute illnesses and minor injuries
- Prescription refills and medication management
- Laboratory services and health screenings
- Patient education and wellness guidance
- Secure electronic health records access via our patient portal
All services are provided in accordance with applicable medical standards and state and federal healthcare laws.
Patient Responsibilities
As a patient of Primary Medical Services, you agree to:
- Provide accurate and complete medical history and personal information
- Inform us of all medications, supplements, and allergies you are taking
- Disclose all existing medical conditions and previous treatments
- Follow medical advice and treatment recommendations provided by our healthcare providers
- Attend scheduled appointments on time or provide notice of cancellation
- Pay for services according to our billing policies and payment arrangements
- Maintain confidentiality of your patient portal login credentials
- Notify us promptly of any changes in your contact information or insurance
- Comply with all office policies and procedures
Your cooperation and honest communication are essential to receiving optimal healthcare.
Billing and Payment
Payment Responsibility
You are responsible for all charges associated with services rendered, regardless of insurance coverage. We will bill your insurance company as a courtesy, but you are ultimately responsible for any amounts not covered by insurance.
Payment Methods
We accept the following payment methods:
- Credit cards (Visa, Mastercard, American Express, Discover)
- Debit cards
- Electronic checks (ACH)
- Cash
- Checks (made payable to Primary Medical Services)
Insurance and Billing
We participate with most major insurance plans. Please bring your current insurance card to your appointment. If your insurance information changes, please notify us immediately. You are responsible for verifying your coverage and understanding your insurance benefits, co-pays, deductibles, and out-of-pocket maximums.
Past Due Accounts
Accounts not paid within 30 days of the invoice date will be considered past due. We reserve the right to suspend services, refer accounts to a collection agency, or take other legal action to collect outstanding balances. Collection actions may include reporting to credit bureaus.
Financial Hardship
If you are experiencing financial hardship, please contact our office to discuss payment plan options. We may be able to arrange a payment schedule that works for your situation.
Appointment Cancellation and No-Show Policy
Cancellation Policy
We understand that appointments sometimes need to be cancelled. Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. Cancellations made with less than 24 hours notice may result in a cancellation fee of $50.
No-Show Policy
If you fail to appear for a scheduled appointment without providing notice, a no-show fee of $50 will be charged to your account. Two no-shows within a 12-month period may result in termination of your patient relationship with our practice.
Late Arrivals
Patients who arrive more than 15 minutes late may be asked to reschedule their appointment to ensure we can provide quality care to all patients.
Medical Records
Patient Access
You have the right to access, review, and receive a copy of your medical records. Requests for records should be submitted in writing. We will provide records within 30 days of receipt of your request. A copying fee of $0.50 per page and a $5.00 administrative fee may apply.
Record Retention
We retain medical records for a minimum of seven years after the date of the last visit. Following this period, records may be securely destroyed unless you request retention or require records for ongoing treatment elsewhere.
Electronic Records
You may access your medical records securely through our patient portal. Your portal credentials are personal and should not be shared with others. You are responsible for maintaining the confidentiality of your login information.
Limitation of Liability
Primary Medical Services provides medical services in accordance with accepted medical standards. However, we cannot guarantee specific outcomes or results. Medical treatment carries inherent risks, and we are not liable for:
- Outcomes or complications that occur despite appropriate medical care
- Adverse reactions to medications or treatments
- Missed diagnoses resulting from incomplete or inaccurate patient information
- Consequences of failure to follow medical recommendations
- Website downtime or technical issues affecting patient portal access
Informed Consent: By accepting treatment at Primary Medical Services, you acknowledge that you have been informed of the benefits, risks, and alternatives to recommended treatments, and you consent to the proposed medical care.
Patient Conduct
We are committed to providing a safe and respectful environment for all patients and staff. We expect all patients to:
- Treat all staff members with respect and courtesy
- Follow office policies and staff instructions
- Refrain from abusive, threatening, or offensive language or behavior
- Not engage in harassment or discrimination
- Not use illegal substances on office premises
- Not carry weapons into the facility
- Maintain appropriate dress and hygiene standards for medical care
Violation of these conduct standards may result in immediate termination of your care relationship with our practice and removal from the office premises.
Termination of Care
Termination by Patient
You may terminate your relationship with Primary Medical Services at any time by providing written notice. We will transfer your medical records to another healthcare provider of your choice. You remain responsible for any outstanding balances on your account.
Termination by Practice
Primary Medical Services reserves the right to terminate the doctor-patient relationship under the following circumstances:
- Non-payment of bills or repeated violations of our payment policy
- Two or more no-shows within a 12-month period
- Abusive, threatening, or disruptive behavior toward staff or other patients
- Violation of office policies or conduct standards
- Failure to comply with medical recommendations or treatment plans
- Actions that compromise the safety of patients or staff
If we choose to terminate care, we will provide written notice with at least 30 days advance notice (or immediately if safety is at risk) and will assist with transferring your records to another provider.
Modifications to Terms
Primary Medical Services reserves the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting to our website. We will update the "Last Updated" date at the top of this page to reflect any modifications. Your continued use of our services following any modifications constitutes your acceptance of the updated terms.
