Is Primary Medical Services accepting new patients?
Yes, we are actively accepting new patients. Dr. Alfonso Martinez-Irizarry welcomes patients of all ages. You can schedule an appointment online at phreesia.me/Scheduling or call (352) 789-5047. New patients can also walk in during clinic hours.
Do you accept walk-in patients?
Yes, walk-ins are welcome during all clinic hours. We see patients on a first-come, first-served basis. For the shortest wait, arrive before 5:00 PM on regular days or before 6:30 PM on evening hours (Monday and Thursday). Learn more at our urgent care walk-in page.

What should I bring to my appointment?
Please bring your insurance card and photo ID. For new patients, arrive 10–15 minutes early to complete intake forms. If you're an established patient, you can pre-complete forms on our patient portal. Bring a list of current medications if you have one.

Where are you located and is there parking?
We're located at 1500 SE Magnolia Ext. Ste. 204, Ocala, FL 34471. Parking is available in the lot adjacent to our office. We are wheelchair accessible. Get directions on Google Maps.
When should I go to the emergency room instead of a clinic visit?
Go to the emergency room or call 911 for chest pain, difficulty breathing, stroke symptoms (facial drooping, slurred speech, weakness), severe bleeding, loss of consciousness, or any life-threatening emergency. Do not wait for a clinic appointment. For urgent but non-emergency concerns, visit us during clinic hours or see our urgent care page.
How do I book an appointment online?
Visit phreesia.me/Scheduling to book online. Select your preferred date, time, and provider. New patients will complete a brief intake form. You'll receive a confirmation email with your appointment details. You can also call (352) 789-5047 to schedule by phone.
Can I request a prescription refill?
Yes. Request refills through our patient portal or call us at (352) 789-5047. Routine refills usually take 24–48 hours. For non-routine refills, Dr. Martinez-Irizarry may request a visit to review your medications.
Who is Dr. Alfonso Martinez-Irizarry?
Dr. Alfonso Martinez-Irizarry, MD is our primary care physician with more than 15 years of clinical experience. He is certified in Anti-Aging Medicine and Bioidentical Hormone Replacement Therapy. He is fluent in English and Spanish and welcomes new patients. Learn more about Dr. Martinez-Irizarry.
Is there a nurse practitioner on staff?
Yes, Mariann L. Lopez, ARNP (Advanced Registered Nurse Practitioner) works alongside Dr. Martinez-Irizarry. She provides primary care visits, wellness exams, and follow-up care. You may request a specific provider when booking your appointment.
Are your evening hours on Monday and Thursday?
Yes, we have extended hours on Mondays and Thursdays until 7:00 PM to accommodate working professionals and families. All other days follow standard hours: Tuesday–Wednesday 8 AM–5 PM, Friday 8 AM–12 PM. Learn more about evening hours.
Why is Friday half-day hours?
Friday hours are 8:00 AM – 12:00 PM to allow our team time for administrative work, lab review, and care coordination. We recommend scheduling Friday appointments early in the morning. For afternoon appointments, Monday, Tuesday, Wednesday, or Thursday are better options.
